This study was conducted in 384 obese children to determine whether metabolic syndrome, defined in three different ways, was associated with arterial alterations predictive of future cardiovascular events. Non-invasive ultrasound was carried out in a subgroup of 161 patients so as to assess arterial mechanical properties and endothelial function. Intima-media thickness positively correlated with LDL cholesterol and negatively correlated with HDL cholesterol. In children aged 11-18 years, cross-sectional vascular compliance negatively correlated with abdominal fat. There was no significant difference in vascular variables between patients with and without metabolic syndrome when using any of the three definitions. In conclusion, metabolic syndrome in obese children was not related to arterial variables whereas several of its individual components were associated with vascular alterations. Based on these findings, the authors put forth that the value of the metabolic syndrome as a condition comorbid with early vascular alterations predictive of future cardiovascular events in children still needs to be evaluated in prospective studies.
